What to Study for the CMSRN Exam: A Guide to the 2023 Blueprint

The Certified Medical-Surgical Registered Nurse (CMSRN®) credential validates a nurse’s knowledge and skills in medical-surgical nursing. The certification is administered by the Medical-Surgical Nursing Certification Board (MSNCB).

If you are preparing for the CMSRN exam, the exam blueprint should guide your study plan. It identifies the knowledge and skills measured on the exam and shows how much of the scored exam is devoted to each domain.

The current blueprint, implemented in 2023, represents a major change from earlier versions. Instead of organizing the exam primarily around physiological systems and nursing functions, the blueprint now uses five domains of medical-surgical nursing practice:

3. Elements of Interprofessional Care
4. Professional Concepts
5. Nursing Teamwork and Collaboration

This guide explains the weight of each domain, the topics you should know, and how to use the blueprint to prepare effectively.

CMSRN Exam Eligibility Requirements

According to MSNCB, candidates practicing in the United States, its territories, or Canada must meet the following requirements at the time of application and throughout the 90-day testing period:

• Hold a current, unencumbered registered nurse license in the United States, a U.S. territory, or Canada.
• Have completed at least 2,000 hours of practice in a medical-surgical setting during the previous three years.

Eligible practice may include working as a clinical nurse, manager, or educator. MSNCB recommends, but does not require, two full years of medical-surgical nursing practice.

International candidates who do not hold a U.S. or Canadian RN license are subject to separate eligibility and credential-evaluation requirements. Review the current CMSRN eligibility requirements before applying.

CMSRN Exam Format

The CMSRN exam contains 150 multiple-choice questions and has a testing time of three hours.

• 125 questions are scored.
• 25 questions are unscored pilot questions.
• The passing standard score is 95.
• A standard score of 95 is approximately equivalent to answering 71% of the scored questions correctly.

The pilot questions are included to collect statistical information for possible use on future exams. They are not identified during testing, so approach every question as though it will count toward your score.

What Is on the CMSRN Exam?

The 2023 CMSRN exam blueprint distributes the 125 scored questions across five domains:

Domain 1: Patient/Care Management

Exam weight: 32% | 40 scored questions

Patient/Care Management is the largest domain on the CMSRN exam. It evaluates how nurses provide safe, evidence-based care across the medical-surgical environment.

Patient Safety

Review how the nursing process supports safe patient care, including assessment, diagnosis, planning, implementation, and evaluation. You should also be prepared to apply safety protocols related to falls, skin integrity, restraints, rounding, suicide risk, equipment, and the care environment.

Important concepts include:

• Patient risk factors
• Near-miss and adverse-event reporting
• Just culture and speaking up about safety concerns
• Care bundles, checklists, and clinical algorithms
• Abuse and human trafficking assessment and reporting
• Social determinants that affect patient safety
• Root cause analysis and failure mode and effects analysis
• Safety rounds and safety huddles

Infection Prevention

Expect questions about standard and transmission-based precautions, infection-control standards, and current evidence-based practices. Review antimicrobial stewardship and the nurse’s responsibilities for preventing health care-associated infections.

Medication Management

Know how to administer medications safely and recognize interactions, adverse reactions, and complications. This topic also includes intravenous therapy, polypharmacy, medication education, safe medication disposal, and management of advanced access devices such as ports, central lines, and epidurals.

Pain Management and Nonpharmacological Interventions

Study acute and chronic pain management, including pharmacological, nonpharmacological, and multimodal approaches. Be prepared to consider patient expectations, preferences, and advocacy when selecting or evaluating an intervention.

Nonpharmacological interventions may include repositioning, heat, cold, acupuncture, aromatherapy, and other complementary therapies.

Surgical and Procedural Nursing Management

Review preprocedural and postprocedural care, informed consent, timeouts, monitoring requirements, potential complications, supplies, equipment, and the nurse’s scope of practice during procedures such as moderate sedation.

Nutrition

Know how disease processes, complications, cultural factors, and individual needs affect nutrition. Review enteral and parenteral nutrition, indications for alternate nutrition, and when to involve resources such as dietitians or speech-language pathologists.

Domain 2: Holistic Patient Care

Exam weight: 15% | 19 scored questions

Holistic Patient Care addresses the patient as a whole person. Questions may require you to incorporate the patient’s goals, values, culture, language, health literacy, family relationships, and end-of-life preferences into the plan of care.

Patient-Centered Care

Review active listening, communication preferences, family involvement, patient advocacy, and available support resources. You should also understand how nurses respond appropriately to grievances, requests for second opinions, concerns about care, and opportunities for service recovery.

Diversity and Inclusion

Study how cultural and linguistic needs affect care. This includes the appropriate use of qualified interpreters, translated materials, and communication accommodations. Be prepared to recognize how implicit bias can influence clinical decisions and patient outcomes.

Education of Patients and Families

Know how to assess health literacy, readiness to learn, and barriers to learning. Select teaching methods that match the patient’s needs and evaluate whether the patient or caregiver understands the information provided.

Health Promotion

Review disease prevention, health maintenance, individualized health goals, and resources that help patients manage their health after discharge.

Palliative and End-of-Life Care

Study hospice and palliative care resources, advance directives, code status, organ donation, postmortem care, and reporting requirements following a death. Questions may ask you to protect patient autonomy while supporting caregivers and coordinating with other professionals.

Domain 3: Elements of Interprofessional Care

Exam weight: 17% | 21 scored questions

This domain focuses on clinical judgment, collaboration, care transitions, documentation, and health care technology.

Nursing Process and Clinical Judgment

Be ready to assess patient information, identify priorities, plan appropriate interventions, evaluate outcomes, and individualize care. Many questions may present a patient scenario and ask for the nurse’s best next action.

Interprofessional Collaboration

Understand the nurse’s role within the interprofessional team. Review interprofessional rounding, care coordination, collaborative problem-solving, and communication among nurses, providers, therapists, pharmacists, case managers, and other team members.

Care Coordination and Transition Management

This area includes:

• Discharge planning and medication reconciliation
• Community resources
• Collaboration with physical therapy and other disciplines
• Collection of health history information from multiple sources
• Care across the health care continuum
• Identification of patients at risk for readmission
• Social determinants of health
• Quality patient-outcome measures

Documentation and Technology

Review accurate documentation, electronic health records, downtime procedures, equipment troubleshooting, nursing informatics, and emerging health care technology. Questions may test both the clinical and legal implications of incomplete or inaccurate documentation.

Domain 4: Professional Concepts

Exam weight: 15% | 19 scored questions

Professional Concepts measures communication, critical thinking, workplace well-being, ethics, quality management, and evidence-based practice.

Communication

Study the chain of command, conflict resolution, mediation, de-escalation, active listening, and barriers to communication. Be comfortable applying structured communication methods such as:

• SBAR
• Hand-off and bedside reporting
• Closed-loop communication
• Check-back and read-back
• Safety huddles
• Verbal-order procedures

Critical Thinking

Expect questions on time management, prioritization, care of deteriorating patients, crisis management, early warning systems, and activation of the rapid response team. Focus on recognizing the most immediate threat and selecting the action that best protects the patient.

Healthy Practice Environment

Review workplace safety, staffing and resource allocation, peer accountability, nurse resilience, and well-being. You should be able to recognize moral distress, moral injury, compassion fatigue, and burnout and identify appropriate responses or resources.

Scope of Practice and Ethics

Know the nurse’s professional and ethical responsibilities, including patient rights, required reporting, unsafe practice, regulatory requirements, licensure rules, and organizational policies. Questions may require you to identify when an issue should be clarified, reported, or escalated.

Quality Management

Study nursing-sensitive indicators, continuous quality improvement, adverse-event reporting, professional practice models, patient-experience data, service recovery, and project development.

Evidence-Based Practice and Research

Review the principles of evidence-based practice, the research process, and the relationship between current evidence, organizational policy, and nursing care. You should understand how nurses locate, evaluate, and apply evidence while remaining within applicable legal and professional standards.

Domain 5: Nursing Teamwork and Collaboration

Exam weight: 21% | 26 scored questions

Nursing Teamwork and Collaboration is the second-largest domain. It covers delegation, supervision, professional development, leadership, and disaster management.

Delegation and Supervision

Review the principles of safe delegation, including scope of practice, staff competency, patient stability, task predictability, supervision, and follow-up. Know which activities may be assigned to licensed practical or vocational nurses and unlicensed assistive personnel within applicable regulations.

Questions may also involve prioritization, staffing, supply use, and fiscal efficiency.

Career Development Relationships

Study mentoring, coaching, reflective practice, professional empowerment, orientation planning, preceptor responsibilities, and career-development resources. Understand how an experienced nurse supports colleagues while maintaining accountability for safe care.

Professional Development

This area includes professional competencies, clinical judgment, peer review, educational-needs assessment, participation in professional organizations, and other behaviors that support continuing professional growth.

Leadership

Leadership topics include:

• Regulatory and compliance standards
• Organizational structure
• Shared decision-making
• Nursing philosophy and leadership models
• Nursing care delivery systems
• Change management
• Recruitment, retention, and employee engagement
• Staff advocacy
• Conflict management
• Financial stewardship

Disaster Planning and Management

Review emergency procedures and the hospital incident command structure. Be prepared to determine nursing priorities, follow the chain of command, allocate limited resources, and communicate effectively during a disaster or facility emergency.

How to Study for the CMSRN Exam

Use the Blueprint to Allocate Your Study Time

The domain weights should influence how you organize your study plan. Patient/Care Management and Nursing Teamwork and Collaboration account for 53% of the scored exam, so these domains deserve substantial attention.

However, do not ignore the three smaller domains. Together, Holistic Patient Care, Elements of Interprofessional Care, and Professional Concepts account for the remaining 47%.

Practice Applying Knowledge to Patient Scenarios

The CMSRN exam is not limited to recalling facts. You may need to analyze findings, set priorities, select an intervention, delegate a task, or coordinate care. When answering a scenario-based question, ask:

• What is the most immediate threat to the patient?
• Which finding requires follow-up first?
• What action is within the nurse’s scope of practice?
• Which task can be delegated safely?
• What information changes the patient’s priority?
• Which response best protects the patient’s rights and preferences?

Study Across Roles and Settings

The blueprint extends beyond direct bedside procedures. Include leadership, delegation, quality improvement, care transitions, documentation, ethics, patient education, and interprofessional collaboration in your review.

Review Every Answer Explanation

When using practice questions, review explanations for both correct and incorrect answer choices. Understanding why an option is inappropriate can help you distinguish between several clinically reasonable responses and choose the one that best answers the question.

Practice Under Timed Conditions

Three hours for 150 questions provides an average of approximately 72 seconds per question. Complete timed practice sessions to build a steady pace while still reading each scenario and answer choice carefully.
