How to Prepare for the NPTE-PT Exam: Format and Content Guide
Passing the National Physical Therapy Examination for physical therapists (NPTE-PT) is an important step toward becoming a licensed physical therapist. Understanding the exam’s eligibility requirements, format, and content areas can help you build a focused study plan and feel more prepared on test day.
What is the NPTE-PT?
The National Physical Therapy Examination is developed and administered by the Federation of State Boards of Physical Therapy (FSBPT). The NPTE-PT is designed to determine whether candidates have the entry-level knowledge needed to practice physical therapy safely and effectively.
The exam focuses on applying physical therapy knowledge, concepts, and principles to patient care. It includes questions covering physical therapy examination, evaluation, differential diagnosis, prognosis, interventions, safety, equipment, professional responsibilities, and evidence-based practice.
Who is eligible to take the NPTE-PT?
According to the NPTE Candidate Handbook, candidates must meet the following requirements:
- Be at least 18 years old
- Submit an online registration and pay the examination fee
- Not have another open registration for the same exam level
- Hold an appropriate degree from a CAPTE-accredited institution or have substantially equivalent education recognized by an approved credentialing agency
- Not have an unresolved exam-security investigation or sanction
- Receive approval from the licensing authority or its authorized designee in the jurisdiction where they plan to become licensed
Licensing jurisdictions may have additional requirements. Review the rules for your jurisdiction before registering.
FSBPT allows eligible students from CAPTE-accredited programs to take the NPTE up to 90 days before graduation, although individual jurisdictions may have more restrictive policies.
How to register for the NPTE-PT
Candidates register for the NPTE through their FSBPT account. Your registration must be complete, payment submitted, and your jurisdiction must approve your eligibility by the applicable deadlines.
Once you are approved, you will receive an Authorization to Test letter and instructions for scheduling your appointment with Prometric.
The NPTE-PT is administered on fixed dates four times per year: January, April, July, and October. Missing a registration, payment, or jurisdiction approval deadline may require you to wait until the next scheduled administration.
NPTE-PT exam format
The NPTE-PT is a computer-based examination administered at Prometric testing centers. Here is what to expect:
- Total questions: 225 multiple-choice questions
- Scored questions: 180
- Unscored pretest questions: 45
- Sections: Five sections of 45 questions
- Testing time: Five hours
- Total appointment time: Five hours and 30 minutes, including the tutorial, scheduled break, and post-exam survey
- Scheduled break: One 15-minute break after the second section
The exam includes stand-alone multiple-choice questions, some of which may contain graphics or videos. It may also contain up to 40 questions connected to shared clinical scenarios. Each scenario includes two to five independent questions.
You can review and change answers within your current section. Once you end a section, however, you cannot return to it.
What is on the NPTE-PT?
The NPTE-PT Test Content Outline, effective January 2024, organizes the 180 scored questions by body system and physical therapy activity.
NPTE-PT body systems
- Cardiovascular and Pulmonary Systems: 22–27 questions
- Musculoskeletal System: 44–54 questions
- Neuromuscular and Nervous Systems: 39–48 questions
- Integumentary System: 8–11 questions
- Metabolic and Endocrine Systems: 4–6 questions
- Gastrointestinal System: 3–6 questions
- Genitourinary System: 2–5 questions
- Lymphatic System: 4–7 questions
- System Interactions: 8–10 questions
Physical therapy activity areas
Questions connected to body systems are also organized across three physical therapy activity areas:
- Physical Therapy Examination: 38–50 questions
- Foundations for Evaluation, Differential Diagnosis, and Prognosis: 51–66 questions
- Interventions: 47–58 questions
Nonsystem content areas
The exam also includes questions that are not assigned to a specific body system:
- Equipment, Devices, and Technologies: 5–6 questions
- Therapeutic Modalities: 4–6 questions
- Safety and Protection: 5–7 questions
- Professional Responsibilities: 4–5 questions
- Research and Evidence-Based Practice: 3–5 questions
How is the NPTE-PT scored?
Your NPTE score is based on the number of questions you answer correctly. There is no penalty for an incorrect answer, so you should answer every question.
Raw scores are converted to a scaled score ranging from 200 to 800. A scaled score of 600 or higher is passing. Because different exam forms may vary slightly in difficulty, the number of questions you need to answer correctly can vary.
What to expect on NPTE test day
Plan to arrive at the Prometric testing center at least 30 minutes before your scheduled appointment. You must bring two qualifying forms of identification:
- A primary government-issued photo ID that includes your name and signature
- A current secondary ID that includes your preprinted name and signature
The first and last names on both IDs must exactly match the name on your Authorization to Test letter. Candidates whose identification does not match their registration may not be allowed to test.
Cell phone use is prohibited throughout the examination, including during breaks. Candidates may access their lockers during a break only for food, drinks, or medication—not study materials or electronic devices.
What happens after the NPTE-PT?
Your exam status is generally posted to your FSBPT dashboard approximately five business days after the examination window ends. A free score report containing your numerical scaled score is typically available approximately 10 business days after your exam date.
If you pass, FSBPT sends your score to the jurisdiction you selected during registration. You must then complete any remaining licensure requirements established by that jurisdiction.
If you do not pass, you may register for another administration as long as you have not reached an FSBPT testing limit. Candidates cannot:
- Test more than three consecutive times within a 12-month period
- Take either exam level more than six times during their lifetime
- Test again after receiving two scores below 400 on two previous attempts
How to prepare for the NPTE-PT
Start by reviewing the official content outline and identifying the body systems and activity areas that require the most attention. The musculoskeletal, neuromuscular, and nervous systems account for a substantial portion of the scored exam, but your plan should still cover all systems and nonsystem categories.
